A colleague of mine related a conversation he recently overheard at the AHCE Congress in Chicago:  The COO of a 350-bed hospital was speaking to a consultant about how to best embrace an "accountable care" model, when the consultant challenged the administrator with an unexpected question; "Does it make sense for you to become an ACO at all?"  The COO confessed that it was almost impossible to satisfactorily account for risk with the current decision-making tools at his disposal.
