Join us for a complimentary 60-minute webinar showcasing how payers and other risk-bearing organizations are implementing proactive strategies that target near-cost savings, quality improvement, and patient engagement opportunities.
The webinar will demonstrate the use of clinical, financial, and predictive analytics to implement—and improve the outcomes of—cost-effective population health management:
• Impactful care management focused on near-term spending for high-risk chronic patients
• Efficient quality initiatives that improve longer-term clinical outcomes and minimize disease progression
• Targeted wellness programs that drive engagement and reduce the impact of lifestyle behaviors on future health and cost metrics
We'll discuss the use of disease registries, HRAs, gaps in care analysis, and risk stratification techniques to fuel care programs tailored to the needs of your population.
